Whether you need help yourself or help caring for someone you love, you can be confident our dedicated team will exceed your expectations. Our home health team incorporates the expertise of registered nurses, home health aides, physical, occupational and speech therapists, certified wound care specialists, medical social workers, certified psychiatric nurses and other professionals. All our caregivers are fully screened and trained, and our aides are also supervised by a nurse case manager.

Hospital stays are often very short. But rest assured, we’re here to lend a helping hand, providing the services, technology and care you need in the comfort and privacy of your home. We offer support for everything from medication education, teaching you or your loved one how to manage a new disease or condition, or teaching you or your family how to manage your own wound care or IV medications. Our services can include:

  • Medical Social Work: Assist patients and families to identify home care need and community resources available to them. Provides counseling and support for coping with complexity of managing chronic illness and identify barriers that inhibit quality of life. MSW’s can assist with respite needs, facility placement, durable power of attorney planning, DNR’s, meals on wheels, medical financial concerns and more to help you stay in your home for as long as possible.
  • Physical, Occupational and Speech Therapy: Physical, occupational and speech therapists use proven rehabilitation techniques to help patients live fuller, more independent lives. Specialty services for rehab include lymphedema treatment, vestibular rehabilitation, pelvic floor rehabilitation, and Vital Stim treatment for swallowing difficulty.
  • Skilled Nursing Care: Hospital-quality care that’s delivered at home. We offer the full range of nursing services and clinical specialties to patients of all ages, including support for managing both chronic and acute medical conditions and a certified wound/ostomy nurse (WOCN) on staff.
  • Telehealth: A simple-to-use device that collects vital signs, such as blood pressure, pulse oximetry, heart rate and weight, and transmits data to Holland Hospital Home Health Telehealth Services. The telehealth team will take appropriate action if a patient’s health status requires intervention.
