Cardiac Rehabilitation and Outpatient Support
Help for recovering cardiac patients and those at risk of heart disease is available through the Cardiac Rehabilitation Program. A medical team of cardiologists, registered nurses, physical therapists, exercise physiologists, respiratory therapists, dietitians and behavioral health specialists develop individualized programs for recovery and lifestyle changes. An ongoing maintenance exercise program also is offered to help patients maintain healthy lifestyles in a supervised setting.
CHEER—Congestive Heart Failure Education Evaluation Resource
Holland Hospital's CHEER Program is a nationally recognized out-patient program that helps patients diagnosed with heart failure understand their condition and increase their quality of life.
This FREE 6-month program is led by registered nurses who work closely with the your physician to help keep you well and out of the hospital.
The CHEER Program includes one-on-one education, support groups, follow up phone calls and home visits that teach important lifestyle changes and monitor patients' well-being, fluid retention and other symptoms. It is intended for those with a history of disease or those who are at risk.
