Experience hospital quality care in the comfort of home. We offer a full range of nursing services and clinical specialties to patients of all ages. Our accredited program employs a highly skilled team of professionals that include RNs, LPNs, MSWs, PTs, OTs, Speech and Language Pathologists and Home Health Aides. All patients are monitored by an RN case manager with on-call services available 24 hours a day.

Acute and Long-Term Care
Coordinated care of frail, elderly, disabled and chronically ill patients—an alternative to nursing home care.

Alzheimer/Dementia Care


Cardiac Care
Nurses specially trained in cardiac care work with the physician and team members to provide care and education.

Diabetic Care
Nursing care and education for glucose monitoring, initiation of insulin therapy, pump therapy and lab draws.

Infusion Therapy
Management of infusion therapies for antibiotics, hydration, pain management and nutrition.

Lymphedema Care
Treatment of lymphedema including education, specialized therapy, exercise instruction and skin care techniques.

Maternal, Infant & Pediatric Nursing
Prenatal and postpartum education and treatment, newborn and pediatric assessments, injections and more.

Medical Social Work
Assessment and counseling on financial problems, community resources, planning, and other adjustment issues.

Oncology Services
High level nursing care, education and rehab for children and adults. Counseling for patient and family.

Psychiatric Nursing & Psych Occupational Therapy
Care for psychiatric disorders, Alzheimer’s, Parkinson’s, depression and difficulty adjusting to life changes.

Rehabilitation
Physical and occupational therapy, speech/language pathology and assistance in establishing a safe home environment for patients with weakness or disabilities related to illness and injuries. As an extension of the Holland Hospital Joint Replacement Center, we provide post operative joint rehabilitation.

Wound & Ostomy Management
Certified wound and ostomy nurses provide care and management of wounds and other non-healing ulcers using the new wound adviser technology which enables photograph transmission of wounds to a certified wound care nurse for the most efficient management and healing.
