What can I expect?
Within a 24 hours of your first call to us, a registered nurse will contact you to arrange an initial assessment. The assessment enables the nurse to become familiar with your specific needs. It includes review of your medical history, physical status, medications and an evaluation of your residence's suitability for various home care services. To achieve the best of care, we work under the direction of your personal physician and will coordinate your needs with your physician. This team approach allows the development of a program specific to you, to ensure that all medications, treatments and therapies are properly administered. Progress is regularly monitored by a registered nurse specializing in home care.
Who actually provides the services in my home?
After your initial visit by a registered nurse, home care services are provided by nurses, home health aides, rehabilitation therapists and/or medical social workers as determined by your professional plan of care. As an extension of Holland Hospital, they are able to provide punctual and reliable service while fostering a familiar, hometown interest in you. Our home care programs are accredited by CHAPS (Community Health Accreditation Program): your assurance that you receive the highest quality of care.

You may call your physician to request a referral, or directly contact our office.
Will my insurance cover the cost?
It may. Please contact our staff for assistance in determining eligibility for coverage through Medicare, Medicaid or Blue Cross/Blue Shield. We will also check with other insurance companies, HMOs or other carriers for details.
How long will I have Home Health Care?
This will vary based on your condition and every patient is different. We will work with you and your physician to develop a care plan that addresses your needs. The frequency of the visits and the duration of care will be dependent on your needs.
What kind of Personal Care Services are available?
Our capable and compassionate staff are available 24 hours, 7 days a week to perform a wide range of support duties including meal preparation, bathing, grooming, housekeeping, transportation, grocery shopping, errands and more. We offer free Rregistered Nurse assessments and case management. Caregivers and aides are fully screened, trained and regularly supervised by a Registered Nurse case manager
Home care clients often need assistance with daily personal care tasks. Our experienced staff can help make sure that regular personal care is accomplished with compassion and dignity. Home Health staff can also help clients maintain a clean, well-organized and safe home environment.

What kind of Respite Care is available?
Those who care for a family member or other loved one need a “break” now and then. Our team provides caregivers with peace of mind and confidence to take time for themselves. We work closely with caregivers and family members to determine the type of services, routines and preferences that will make the client feel most comfortable and secure.
