Going Home

When your doctor decides you are ready to leave the hospital, a discharge order will be written. You may want to make arrangements with a family member or friend to help you when it’s time to leave. When you are ready to leave, your nurse will call the volunteer Errand and Escort Service for a volunteer to escort you to your car.
Discharge Planning
Holland Hospital offers this service to plan for your release from the hospital. Your RN care manager will meet with you to determine your needs and discuss any concerns you might have about going home. These concerns may include physical, psychosocial, medical and equipment needs, or financial and insurance questions.
Personal Belongings
Please collect all your belongings and check closets and drawers. If you have items stored in the hospital safe, please ask your nurse to get the items for you. If after returning home you realize you have left something at the hospital, please call
Security Services at (616) 394-3736.
Post-Hospital Care
There are occasions when patients need additional treatment or care after they are discharged from the hospital. To meet the special needs of these patients, Holland Hospital offers a number of post-hospital care options.
Home Health Services
Medical services provided in the home, including skilled nursing care, physical therapy, psychiatric care, infusion therapy, wound care and care of ventilator-dependent patients. More >
Meals on Wheels
Holland Hospital’s Meals on Wheels is a community-service program ponsored by the hospital’s Food Services Department. The program provides home-delivered lunches to convalescents, disabled individuals, and elderly persons who are unable to prepare adequate meals for themselves. The lunch meals are offered at very reasonable costs, are prepared under the direction of the hospital’s Food and Nutrition Services, and meet the dietary needs recommended by the customer’s physician. Lifeline
The Holland-Zeeland Lifeline program is a 24-hour, personal emergency-response system that provides immediate access to at-home assistance in an emergency situation. The program, a cooperative effort offered by Holland and Zeeland hospitals, helps elderly and disabled people maintain their independence. While in the hospital, you may request that a representative meet with you in your room. If your concern is immediate and/or you are not comfortable talking with your nurse, call the hospital operator (dial 0) and ask to speak with the Patient Care Coordinator.
Shortly after your stay, you may receive a phone call from Arbor Associates, a firm Holland Hospital contracts with to conduct random telephone surveys of patients to learn about their experience and satisfaction. If you receive this survey or phone call, we appreciate your time and cooperation. Your feedback will be very helpful to us in evaluating our services and staff
