Employee Wellness Program

Holland Hospital is pleased to offer LIFE Rewards as part of our Employee Wellness Programs for 2009/2010. Our partnership with WebMD provides you with your own private health site along with HealthQuotient (HQ) to help track your health progress.

The LIFE Rewards program begins on April 1, 2009, and ends on March 1, 2010. Print off the LIFE Rewards Booklet for all the details or request a copy from Employee Wellness.

Incentives

Complete all six requirements at 25 points each for a total of 150 points and receive your $150 check on March 31, 2010 (check is live, included as income and subject to taxes). Print off the LIFE Rewards At-A-Glance and the LIFE Rewards FAQ - Directions.

LIFE Rewards Program Requirements

Update your HealthQuotient (HQ) via WebMD.

Returning users: Please login using your private username and password to access and update your HQ.
New users: Please register using your six digit Employee ID number.

Complete your free lab work.

Wellness lab draws are available between June 1, 2009, and March 1, 2010.

If you are pregnant, you may omit the lab draw and read five recommended articles. Print off the LIFE Rewards FAQ - Pregnancy.

Complete your biometric screening.

This screening includes your height, weight, waist measurement, blood pressure, pulse and percent body fat.

Complete annual physical/recommended annual care.

Employee spouses are encouraged and welcome to join the LIFE Rewards Program. Spouses will receive free use of WebMD, free lab work and biometric screening.
