Employee Wellness Program
LIFE (Lifestyle Improvement for Employees) Rewards is a part of Holland Hospital’s comprehensive Employee Wellness program. It empowers employees to take responsibility for healthy lifestyle choices, actively work toward better health and engage in behaviors that develop optimal health. Program deadline for completion is March 1, 2009.
Incentives! Complete the six listed requirements below and receive $150 check, payable March 31, 2009. (This money will be taxed and counted as income.)
- Get registered! Complete the online HealthQuotient® tool powered by WebMD. You must use your six-digit employee ID number to register – look in Kronos or on your paycheck stub. If you are already a registered user, log in and update your HealthQuotient risk assessment profile. Print an Employee Wellness Booklet

- Complete your biometric testing
– dates, times and locations to be announced.
- Complete annual physical/recommended annual care.
- No tobacco use by March 1, 2009.
- Set one Wellness Goal and achieve it. Log into WebMD and read about SMART goal setting on your personal home page.
*Spouses of employees are welcome and encouraged to join the program, including free lab work and free use of the WebMD tools. Spouse Enrollment Form
