For people living with diabetes, learning to self-manage their condition is critical to their success in healthy living.

Holland Hospital offers the Diabetes Self-Management Education Program to help you successfully manage your diabetes. The program is based on national standards of the American Diabetes Association. Successful diabetes management is a team effort that includes you and your family, your physician and your diabetes educators. The goal of the program is to provide individuals and their families with the tools to take control of diabetes, safely manage the disease and prevent serious complications.   • Meal Planning for blood sugar control
  • Improving blood sugar through physical activity
  • Monitoring blood sugar levels and targets to reach
