Holland Hospital is dedicated to providing comprehensive, ongoing support to recovering cardiac patients and those at risk of heart disease. Our team of cardiac rehabilitation experts,  as well as our nationally recognized CHEER Program, will help you make the lifestyle changes necessary to improve your heart health and longevity.
Help for recovering cardiac patients and those at risk of heart disease is available through the Cardiac Rehabilitation Program. We develop an individualized program for your rehabilitation and offer an ongoing maintenance exercise program to help you maintain a healthy lifestyle in a supervised setting.

Our dedicated rehabilitation team includes:

  • Physical Therapists
  • Exercise Physiologists
  • Respiratory Therapists
  • Dieticians
  • Behavior Health Specialists

Cardiac Rehabilitation is a comprehensive program consisting of three phases:
Phase I Inpatient:  This phase begins during your hospital stay and provides education about the heart and good health habits.  Low intensity exercise also begins at this time.
Phase II Outpatient:  This phase is a program of education, exercise and fun generally lasting eight to twelve weeks.  The health care team helps you set realistic goals and take an active role in your recovery.  Exercise sessions meet three times per week and include continuous supervision and heart monitoring.  Staff also provides and suggests dietary and stress management modifications.
Phase II Ongoing:  This phase provides regular exercise supervision and support for continued positive lifestyle changes.  You may chose to participate in this phase on an ongoing basis.

CHEER Program

CHEER is an educational, out-patient support program to help you adjust to life with heart failure. This nationally recognized program helps you understand your condition and increase your quality of life.

The CHEER program is a free service of Holland Hospital for patients with a history of disease and for those at risk. The 6-month program is led by registered nurses who specialize in heart failure and who works closely with your doctor to monitor your condition.

The CHEER Program includes:

  • Home visits which teach you important lifestyle changes and educate you about your medications
  • Printed materials to reinforce the home-visit education
  • Phone calls to monitor your progress
  • Office visits
  • Access to nurses for any questions you may have during business hours
  • Monthly support groups to help you and your family adapt to your new lifestyle changes
