When your doctor decides you are ready to leave the hospital, a discharge order will be written. You may want to make arrangements with a family member or friend to help you when it’s time to leave. When you are ready to leave, your nurse will call the volunteer Errand and Escort Service for a volunteer to escort you to your car.

 

Discharge Services

Holland Hospital offers this service to plan for your release from the hospital. Your RN care manager will meet with you to determine your needs and discuss any concerns you might have about going home. These concerns may include physical, psychosocial, medical and equipment needs, or financial and insurance questions.

 

Personal Belongings

Please collect all your belongings and check closets and drawers. If you have items stored in the hospital safe, please ask your nurse to get the items for you. If after returning home you realize you have left something at the hospital, please call Security Services at (616) 394-3736.

 

Post-Hospital Care

There are occasions when patients need additional treatment or care after they are discharged from the hospital. To meet the special needs of these patients, Holland Hospital offers a number of post-hospital care options.

 


Home Health Care

Holland Hospital Home Health Services offers excellent medical care in the comfort of your home. Our skilled team includes registered nurses, home health aides, medical social workers, nutrition consultants, physical and occupational therapists, speech pathologists and others.

 

Holland Hospital Home Health Services offers a variety of personal care services in addition to skilled in-home nursing care to meet short or long term needs. Our capable and compassionate staff are available 24 hours, 7 days a week to perform a wide range of support duties including meal preparation, bathing, grooming, housekeeping, transportation, grocery shopping, errands and more. We offer free RN assessments and case management. Caregivers and aides are fully screened, trained and regularly supervised by an RN case manager.
