Planning for discharge from the hospital begins when surgery is scheduled. Your home situation, mobility, medical condition and the availability of family assistance may influence the discharge plan.
While each patient is different, the goal is for you to recover from your joint replacement surgery in the comfort of your own home as soon as possible. Most patients prefer to have family members and friends close-by to support them during rehabilitation.
If special assistance such as medical equipment rental, home nursing care, outpatient therapy or a stay in a skilled nursing facility are required, our Care Management department will work with you to coordinate the details.
